The in-take form is very easy to fill out. Information on every page isn't relevant to everyone and you'll most likely skip through most of it. Each page of the form is focused on a specific area:

Page 1 is about you (demographic info)
Page 2 is about your Undergraduate Degree
Page 3 asks about your Masters (if you don't have one skip)
Page 4 speaks to your PhD experience (if you don't have one skip)
Page 5 asks about Medical Degrees, Page 6 about Law, Page 7 about additional educational experience
(again, if you don't have these, you can skip)
Page 8 asks about employment
Page 9 speaks to your professional/social organization membership, and
Page 10 asks questions about possible activities on BSI
